Complete and submit the following documents to the general contractor

• Section 3 Requirements | Understanding and Commitment Statement

• Section 3 Workforce Commitment Statement

 

CMHA Construction Documents

 

Complete and submit the following documents to the general contractor

• Available Job Opportunities Form (required when new employment or training opportunities exist)

• Section 3 Business Certification Form (required when contractor is seeking confirmation as a Section 3 business)

• Sample Employee Other Deduction Authorization Statement (may be used and submitted with payrolls for other deductions authorization)

 

Elation Systems: Registration Instructions (instructions to register for a free Elation Systems account required to submit certified payrolls)

Elation Systems: Section 3 Documents Handling Input/Upload Instructions

 

Submit the following to Elation Systems

• Core Employee List (input data in Elation Systems; required before the submission of payroll 1; do not revise)

• 2024 Section 3 Worker Certification Form (required for each worker appearing on certified payrolls; READ INSTRUCTIONS)

 

General Contractor Documents

 

Complete and submit the following documents to the general contractor per instructions provide by the GC

• Contractor Profile Form

• Payroll Dates Form

• Fringe Benefits Form

• SMSD Form

• Final Affidavit
